Interim Report

The Comis Foundation requires all grantees to complete both an Interim Report and a Final Report. The form below is to submit an INTERIM Report. To submit a FINAL Report go to the the FINAL Report form.

The purpose of the interim report is to assess progress in meeting the goals of the project and to alert both the grantor and the grantee to unforeseen obstacles that may require redefinition of the project or a change in its timescale. Usually the award period is one year, and the interim report should be submitted to Comis no later than nine months from the award date.

Complete and submit the following form. Note that your responses are intended to be brief; each question allows for only 500 characters of text – roughly 75 words per question.
