The Comis Foundation requires all grantees to complete both an Interim Report and a Final Report. The form below is to submit a FINAL Report. To submit an INTERIM Report go to the the INTERIM Report form.
A final report is due one month after the end of funding as noted in the Grant Award letter. Final reports have three purposes: First, they assist the Foundation with developing and maintaining standards for funding work of high quality. Second, they establish the ability of the grantee to produce results. Third, they provide a record for financial accountability.
